Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to deliver skilled nursing care in the home. You will follow physician orders, administer care per the plan of care, and educate patients and families to support health outcomes.

Candidates must hold a valid state license and have at least one year of relevant experience, with CPR certification and comfortable use of mobile devices for documentation and coordination.

Qualifications

• Graduate of an approved school of clinical education.
• Must be licensed in the state where they practice.
• At least one year of experience as a licensed professional required.
• CPR certification and basic technology skills including mobile device use required.

Responsibilities

• Administer skilled care to patients per physician orders and the plan of care.

Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

• Must be a graduate of an approved school of clinical education.


• Must be licensed in the state where they currently practice.


• Must have at least one year experience as a licensed professional.


• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.


• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license


• Must maintain automobile liability insurance as required by law


• Must maintain dependable transportation in good working condition


• Must be able to safely drive an automobile in all types of weather conditions


• Must possess CPR certification for the healthcare provider
