Job summary

Enhabit Home Health & Hospice Careers is seeking a qualified clinician to administer skilled care to patients in the home setting and to educate patients and families as part of the care team.

The role requires graduation from an approved clinical education program, state licensure, and at least one year of licensed professional experience. A valid driver’s license and reliable transportation are required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Education and experience, preferred.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

This salary range includes the potential to earn over productivity bonus and on-call pay

30 days PDO – Up to 6 weeks (PDO includes company observed holidays)

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
