Professional Case Management is hiring in Prosperity, SC for in‑home RN positions (part-time and full-time). You’ll deliver patient‑centered care in clients’ homes on a schedule that fits your life, with weekly pay and the satisfaction of making a direct difference.

Responsibilities include assessing health status, reviewing care plans, administering prescribed medications, and maintaining a safe home care environment. Required: SC RN license, CPR, TB test; BSN preferred; new grads welcome.

Essential Functions:

• Assess and evaluate health status of assigned clients
• Review plan of care to evaluate client response to interventions

• Perform job in compliance with Agency policies, procedures, and community standards
• Practice nursing with respect for individual, cultural, and spiritual differences
• Possess strong communication and interpersonal skills

Standard Rate: $40.00 - $42.50 Hourly
