Job summary

Enhabit Home Health & Hospice is seeking a licensed home health nurse to provide skilled nursing care in patients' homes. You will assess, plan, implement, and evaluate care in collaboration with a multidisciplinary team, and educate patients and families on treatment plans.

The role requires an active state license, a valid driver’s license, and at least one year of licensed professional experience. Medicare home health or hospice experience is preferred, with mobility and technology

Qualifications

• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

Responsibilities

Education and experience, preferred

• Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.
• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
