Job summary

Enhabit Home Health & Hospice in La Grande, Oregon is seeking an experienced Registered Nurse to provide in-home patient care. You will coordinate care across disciplines and oversee visit frequency to ensure quality outcomes.

The role requires accurate OASIS collection, current medication profiles, timely lab value reporting, and active participation in weekly case conferences and monthly case manager meetings.

Qualifications

• RN license in the state of employment is required.
• Minimum of one year of clinical experience preferred.
• Knowledge of current nursing practice and patient care standards.
• Must possess a valid state driver's license and automobile liability insurance.

Responsibilities

• Perform home visits and coordinate client care.
• Act as the point of contact for all disciplines involved in patient care and oversee visit frequency.
• Consult with physicians and office staff regarding patient care details.
• Perform accurate OASIS data collection and maintain current medication profiles.

Responsibilities

In addition to performing visits and completing coordination of client care, the Registered Nurse RN Home Health is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The Registered Nurse RN consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.

Qualifications

Registered Nurses (RNs) must meet the following requirements:

• Be currently licensed as a Registered Nurse (RN) in the state of employment
• A minimum of one year of clinical experience is preferred
• Demonstrate knowledge and skill in current nursing practice
• Possess a valid state driver’s license and automobile liability insurance
