Job summary

Enhabit Home Health & Hospice in Missouri seeks licensed healthcare professionals to provide skilled home health services under physician orders and the plan of care. You will assess and treat patients in their homes, coordinating with the care team to ensure safe, effective care.

You're expected to teach patients and families, maintain accurate documentation, and uphold quality standards. A valid state license, CPR, reliable transportation, and at least one year of licensed experience are

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
