Directs and manages the care of designated patients. Utilizes effective and appropriate communication styles. Assists in maintaining a safe work environment and maintains clinical and professional competency as appropriate to the population of patients served. **Job Duties*** Performs a comprehensive subjective and objective assessment using standardized assessment tools to gain pertinent details about the patient's overall health, well-being, abilities, and living environment; performs on-going assessment of physical, psychosocial, and environmental parameters.* Delivers patient care based on the medical plan of treatment established by the physician and protocols using a patient family centered approach.
