Job summary

PruittHealth is seeking a Home Health Registered Nurse (RN) to join our patient‑centered team in South Carolina. As a Home Health RN Case Manager, you will deliver skilled nursing visits in patients' homes and develop start‑of‑care plans.

You will collaborate with the interdisciplinary team, supervise LPNs and aides, and document care using an iPad with AI assist. A valid RN license, CPR, and reliable transportation are required; a year of home health or acute care is preferred.

Qualifications

• Active, unrestricted RN license.
• CPR certification is required.
• A valid driver’s license and reliable transportation.
• At least 1 year of RN experience; home health or acute care preferred.

Responsibilities

• Provide skilled nursing visits in the patient’s home.
• Complete assessments, start-of-care visits, and care plans.
• Collaborate with the interdisciplinary care team.
• Supervise LPNs and Home Health Aides.
• Document care using an iPad with AI assist; OASIS experience is a plus.
• Educate patients and families to promote independence.
• Ensure continuity, quality, and safety of patient care.
