The Registered Nurse augments staff knowledge and attitudes as they relate to geriatric care. Conducts informal monitoring of patients > 65 years of age. Assists staff in accessing, planning, implementing, and evaluating geriatric care according to NICHE protocols. Evaluates and provides feedback to staff regarding NICHE Protocols. Communicates geriatric issues in report and rounds. Consults with GEM Team & other staff regarding geriatric care issues. Promotes collaboration with interdisciplinary staff in planning and promoting alternative practices in geriatric care management. Collaborates with the GEM Team, Nurse Manager/Director, NICHE. Facilitates communication between care providers and NICHE committee members. Teaches patients and families about geriatric care issues. Disseminates information about geriatric care management through a variety of ways including coordination of unit in-services, journal article reviews, and dissemination of NICHE protocols. Identifies the need for and assists in the development of geriatric patient education materials. Educates nurses and interdisciplinary staff about geriatric issues through in-service presentations and direct consultation/support at the bedside. Attends NICHE meetings, Geriatric/NICHE in services, and geriatric continuing education programs. Promotes awareness of hospital and community resources for the geriatric patient; GEM Team (e.g., SPICES Tool, Mini Mental State Exam, Geriatric Depression Scale). Assists in updating geriatric related policies and procedures, conducts assessment utilizing best practice geriatric assessment tools, and other related tasks as needed. The nurse will be part of the North GEM team and the position is located on the north side in Indianapolis, Indiana.
