Job summary

Avamere Rehabilitation of Park West in Seattle seeks an experienced Registered Nurse to provide comprehensive patient care, including assessments, med administration, and coordination with the care team. You will supervise LPNs and CNAs, document care, and support families while upholding high standards of safety and quality in a skilled nursing setting.

The position requires an active RN license in Washington, a nursing degree, CPR/BLS, and prior long-term care experience is a plus.

Qualifications

  • Must possess a nursing degree from an accredited college or university.
  • Must hold an active RN license in the state.
  • Current CPR/BLS certification is required.
  • Experience in long-term care or geriatric settings preferred.

Responsibilities

  • Assess residents and develop individualized care plans with the interdisciplinary team.
  • Administer medications and monitor patients according to physician orders.
  • Monitor vital signs and document health status and responses to treatment.
  • Supervise LPNs and CNAs; provide orientation and training as needed.
  • Maintain accurate care documentation and communicate with families and staff.
  • Participate in quality improvement and stay current with best practices in skilled nursing.

Status: Full-Time

Schedule: Tursday, Friday & Saturday or Thursday, Friday & Sunday, 6:00 AM-6:00PM

Wage: $50.00 - $60.00 hourly DOE

Location: Avamere Rehabilitation of Park West - 1703 California Ave SW, Seattle, WA 98116

The primary responsibility of the Registered Nurse (RN) is to provide comprehensive patient care, including assessing residents, administering medications, monitoring health, and coordinating care with other healthcare professionals, while also supervising nursing staff.

Essential Duties and Job Responsibilities

  • Patient Assessment and Care Planning:
  • Conduct thorough assessments of residents' physical, cognitive, and emotional needs.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team, including physicians, therapists, and other healthcare professionals.
  • Regularly evaluate and update care plans based on residents' changing needs and responses to interventions.
  • Medication Administration and Monitoring:
  • Administer medications and treatments as prescribed by physicians, ensuring accuracy and safety.
  • Monitor residents' responses to medications and treatments, documenting any adverse effects or changes in condition.
  • Vital Signs and Health Monitoring:
  • Monitor and document vital signs, such as temperature, pulse, respiration, and blood pressure.
  • Observe residents for any signs of illness or changes in condition and promptly report any concerns to the physician.
  • Wound Care and Other Specialized Care:
  • Collaborate with wound care specialists or other healthcare professionals to ensure optimal wound healing.
  • Supervision and Training:
  • Supervise and provide guidance to licensed practical nurses (LPNs) and certified nursing assistants (CNAs).
  • Provide orientation and training for new staff members.
  • Communication and Documentation:
  • Maintain accurate and complete documentation of residents' care, including assessments, interventions, and evaluations.
  • Communicate effectively with residents, families, and other healthcare professionals.
  • Participate in interdisciplinary team meetings to coordinate care and address resident needs.
  • Quality Improvement:
  • Participate in quality improvement initiatives to ensure the delivery of high-quality care.
  • Stay current with best practices and evidence-based guidelines for nursing care in skilled nursing facilities.
  • Must possess a nursing degree from an accredited college or university.
  • Must possess a current, unencumbered, active license to practice as an RN in this state. BSN preferred.
  • Must maintain and have an active CPR/BLS during employment.
  • Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
  • Experience in electronic medical records documentation.
