Westmoreland Care & Rehabilitation Center, a Signature HealthCARE facility, seeks an experienced RN to provide skilled nursing care and supervise nursing staff in a 100-bed setting. You will implement care plans, monitor patients, and ensure documentation accuracy.

Requires active RN license, 1+ year of clinical nursing experience (long-term care preferred), and supervisory experience is a plus. Excellent communication and EHR skills are required. Equal opportunity employer.

Qualifications

• Active RN license in the state of practice.
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.

Responsabilités

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents’ condition and notify physician/party of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate admissions, transfers, and discharges according to procedures.
• Participate in quality assurance activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.

How you Will make a Difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.

• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
