Works under the direction of leadership and a multi-disciplinary team to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans. **What you will do** * Demonstrates ability to assess patient clinical ands psychosocial needs by applying standards of practice as it relates to the specialty or patient population served in the Critical Care/Telemetry units/departments (see unit specific competencies and equipment checklists).* Partners with patients and families to develop, implement and evaluate plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient. Assesses and incorporates evidence-based standards of practice. Collaborates with interdisciplinary team in the coordination care. Facilitates safe patient transitions to the next care setting as part of plan of care.* Demonstrates understanding of diverse patient care populations and ability to modify education method to ensure patient/family comprehension. Educates by assuming responsibility to individualize and coordinate patient education, and discharge specific education that includes family/caregiver. Evaluates for patient comprehension and documents.* Ability to delegate and prioritize tasks appropriately to facilitate quality patient care and ensure patient care needs are met.
