Concierge Home Care in Port Orange, FL is hiring a Weekend Home Health RN. Earn $80K–$95K+ annually with a 4-day schedule (Wednesday–Saturday OR Sunday–Wednesday) and a 2-day weekend option.

This role offers 1:1 patient care with full autonomy and the flexibility to design your daily schedule. You will perform comprehensive home assessments, develop care plans, and coordinate with physicians, while leveraging WellSky/Kinnser EMR for documentation.

Qualifications

• Active Florida RN license required.
• Valid driver’s license and reliable transportation.
• Home health experience preferred, but not required.

Responsibilities

• Perform comprehensive in-home patient assessments and education.
• Develop and manage individualized care plans.
• Provide skilled nursing care and caregiver education.
• Administer treatments and monitor patient condition.
• Coordinate care with physicians and interdisciplinary teams.

Your Role as a Home Health RN

• Perform comprehensive patient assessments in the home setting
