Job summary

ChristianaCare HomeHealth in Delaware is seeking a compassionate Home Health RN to provide one-on-one skilled nursing in patients' homes across Middletown, Odessa, Bear and surrounding communities. You will deliver prescribed treatments, promote independence, and develop patient-centered care plans.

You’ll collaborate with physicians and caregivers to ensure smooth transitions, manage IV therapy, and support discharge planning while enjoying autonomy and strong professional support.

Qualifications

• BSN required for position.
• Current Delaware RN license or Compact State license.
• Minimum of one year RN experience.
• IV proficiency or willingness to complete training.

Responsibilities

• Deliver skilled nursing care and prescribed treatments in geriatric patients' homes.
• Build trusting relationships with patients and caregivers to promote independence and quality of life.
• Develop and implement individualized, patient-centered care plans aligned with available resources.
• Coordinate care with physicians, caregivers, and interdisciplinary team members to achieve outcomes.
• Develop and execute discharge plans ensuring continuity of care.
• Perform IV therapy and related nursing functions; training provided to achieve IV proficiency within 90 days.

The successful candidate will support patients throughout Middletown, Odessa, and Bear, Delaware and surrounding communities while working day shift hours, 3 weekends per month, two holidays per year, and two on-call shifts per month.

What You'll Do

As a HomeHealth RN, you will:

• Deliver skilled nursing care and prescribed treatments to geriatric patients in their homes and throughout the community.
• Build trusting relationships with patients and caregivers while promoting health, independence, and quality of life.
• Develop and implement individualized, patient-centered care plans that are goal-oriented and aligned with available resources.
• Coordinate care with physicians, caregivers, and interdisciplinary team members to achieve the best possible outcomes.
• Develop and execute discharge plans that ensure continuity of care.
• Perform IV therapy and related nursing functions. New team members will receive training and are expected to achieve IV proficiency within 90 days of hire.

Required Qualifications

• Bachelor of Science in Nursing (BSN) required.
• ADN candidates will be required to obtain a Bachelor of Science in Nursing (BSN) within three (3) years of their hire date, as outlined in the required Education Agreement.
• Current Delaware RN license or Compact State RN license.
• Minimum of one (1) year of RN experience.
• IV proficiency or willingness to complete training and perform IV-related functions.

Preferred Qualifications

• Previous Home Healthcare experience.
• Previous Medical-Surgical nursing experience.

Hourly Pay Range: $30.95 - $49.52

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
