Job summary

Avera Endocrinology & Diabetes Sioux Falls is seeking a Registered Nurse to support urgent care management of blood glucose levels, provide diabetes education, and collaborate with a multidisciplinary team. The role involves coordinating patient care with providers and the diabetes team, and ensuring accurate documentation.

Typical schedule is Monday-Friday, 7:30am-5:00pm with 80 hours per biweekly period and potential eligibility for a sign-on bonus.

Qualifications

• Active RN license in practicing state.
• Diabetes patient care and/or education preferred.
• Ability to communicate effectively and provide patient education.

Responsibilities

• Provide general nursing care and administer prescribed treatments.
• Triage calls, monitor blood glucose lines, and instruct patients and families on treatment plans.
• Educate patients on diabetes self-management per ADA guidelines and provider instructions.
• Collaborate with diabetes team and other clinic staff to coordinate care.

A Brief Overview
Assists providers with patients that are needing urgent care of managing their blood glucose levels. Examples of this would be patients with high or low blood sugars, patients that are scheduled to have surgery, patients who may be put on a steroid medication, and patient needing assistance with diabetic device.
What You Will Do

• When needed performs general nursing care to patients. Administers prescribed medications and treatments in accordance with nursing standards.
• Responds to and triages incoming telephone calls. Monitors the blood sugar phone line and email. Instructs patients and family regarding medications and treatment plans given by provider.
• Assesses patient learning needs and provides diabetes self-management education consist with appropriate principles of teaching/learning and according to American Diabetes Association Recognition requirements and the patient’s health care provider.
• Communicates with providers, diabetes team members, other health care agency staff and nursing staff regarding the care and education of diabetes patients.
• Maintains and views patient charts to obtain the most accurate information the clinic has for the patient as well as updates records as approved by the provider.
• Maintains proficiency on all diabetes devices ie. CGM, meters, and insulin pump.
• Serves as a clinical resource for nursing staff and providers related to the care and education of diabetes patients.
