Job summary

Qualifications

• Current and unrestricted RN license in Indiana.
• At least 1 year of nursing experience; home health/Medicaid preferred.
• Solid understanding of Indiana Medicaid policies, documentation standards, and prior authorization processes.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong clinical judgment and communication/organizational skills.
• Familiarity with electronic documentation systems.

Responsibilities

You'll partner closely with patients and care teams to carry out personalized care plans that promote health, independence, and a better quality of life.

Key Responsibilities:

• Provide skilled nursing services, including wound care, medication administration, chronic disease management, and patient education.
• Ensure all services are medically necessary and properly documented to meet Medicaid reimbursement standards.
• Coordinate care with physicians, case managers, and other members of the interdisciplinary team.
• Empower patients and caregivers with knowledge on disease processes, medication regimens, and self-care strategies.
