Bluestem Communities Management is seeking a full-time Registered Nurse (RN) Clinic Float to support Bluestem PACE participants across McPherson and Hutchinson, KS. The role centers on comprehensive assessments, care coordination, and timely nursing documentation within a dynamic, multi-clinic team.

The RN will administer medications, provide health education, and collaborate with physicians and the care team.

Qualifications

• Must be at least eighteen years old.
• Must have an active Kansas RN license.
• Must have completed an RN training program with diploma from an accredited school of nursing, college or university.
• Must have working knowledge of Microsoft Office Suite.

Responsibilities

• Conduct initial comprehensive assessments and participate in IDT.
• Coordinate care and monitor Bluestem PACE participants; manage medications and routine monitoring.
• Administer and monitor medications; document progress notes and nursing care plans.
• Provide health education and counseling to participants and caregivers facing chronic conditions and end of life issues.
• Liaise with primary care providers and coordinate services.
• Ensure timely, accurate documentation and maintain participant confidentiality.
• Participate in Case Management and quality assurance activities.

This position floats between our McPherson and Hutchinson locations, the person hired for this job must be willing and able to travel between clinics. The hours for this position are Monday through Friday from 8:00am - 4:30pm with a rotating on call schedule.

Position Overview

Under the supervision of the Clinical Service Director and Bluestem PACE Medical Director, the RN collaborates with other PACE providers. The RN is responsible for: assessing, coordinating, monitoring, and providing health care services and treatment for Bluestem PACE participants; and coordinating quality improvement development, support, data collection and analysis.

Essential Job Functions

• Conduct initial comprehensive assessments, unscheduled nursing assessments and participates in IDT.
• Facilitates the integration of new participants into the Bluestem PACE health care delivery system including medication, immunizations, and routine monitoring of chronic problems through the health record and nursing care plan development.
• Provides a high level of customer service to the participants, families and other employees.
• Provides ongoing nursing assessments on a scheduled basis and submits PACE data requirements for caseload participants.
• Monitors and delivers routine nursing care; documents progress notes and nursing care plans on a regular basis.
• Provides liaison with primary care provider in the event of an episodic illness; assists in coordinating services provided by primary care provider.
• Follows established standards of nursing practices and implement program policies and procedures.
• Administers and monitors participants’ medications prescribed by the physician.
• Assists participants in maintaining optimum health; provides health education and counseling to participants and caregivers facing chronic conditions and end of life issues.
• Assures nursing services are documented in a timely, legible and accurate manner.
• Document care in the participant’s medical record consistent with documentation policies and workflows.
• Maintains confidentiality of participant information.
• May provide basic first aid to agency employees as required by incident or injury.
• Actively participates in Case Management. Serves as a member of the interdisciplinary team; participates in the development of the plan of care. Provides updates to team regarding participants’ change in health or functional status.
• Responsible for participant safety, identifying safety hazards and emergency situations and initiates corrective action as able.
• Attend and participates in staff meetings, in-services, projects and committees as assigned.
• Follows, supervises and evaluates the implementation of the Participant’s Bill of Rights.
• Assumes responsibility for compliance with federal, state and local regulations.
• Reports changes in the participants’ conditions to a physician or responsible party promptly and takes follow-up action as necessary.
• Follows Bluestem PACE admission, discharge, and transfer procedures and coordinate admissions, discharges and transfers with other departments, physicians, and ancillary service providers.
• Participates in monthly quality assurance.
• Prepares and administers medications, enteral nutrition, parenteral nutrition and treatment as ordered.
• Assists in assuring personal and staff compliance with infection control, personal protection equipment, blood borne pathogens, hazardous material handling and fire safety.
• Adheres to and supports the Bluestem PACE’s policies, practices and procedures.
• Accepts assigned duties in a cooperative manner and performs all other related duties as assigned.
• Maintains a neat, clean professional appearance.
• Must be flexible in schedule of hours worked.
• Must be able to be on-call
• May require use of personal vehicle
• The RN may be required to perform duties of the CNA, CMA or other nursing staff on occasion.

Qualifications and Experience

• Must be at least eighteen (18) years of age
