Job summary

PruittHealth Hospice is seeking a Registered Nurse Case Manager for on-call coverage in Georgia, to deliver patient-centered hospice care in home or facility settings. You will perform evaluations, develop care plans, and coordinate services with a multidisciplinary team.

Responsibilities include timely documentation, adherence to policies, and maintaining continuity of patient care. Active RN license in the state and experience in hospice or home health are preferred.

Qualifications

• Active RN license in the practicing state.
• Hospice or home health experience preferred.
• Excellent communication and coordination skills.
