Job summary

Blue Ridge Care is seeking a compassionate Registered Nurse Case Manager to join our hospice team in Frederick/Clarke County. You will provide direct patient care, coordinate care plans, and support families with a focus on body, mind, and spirit.

You will lead hospice aides, maintain documentation, and collaborate with an interdisciplinary team. A Virginia RN license and CPR are required, with 2+ years in hospice or home health preferred.

• Current RN license in Virginia (multi-state or single-state).
• CPR certification and at least two years of nursing experience (hospice or home health preferred).
• Strong clinical judgment, communication, and organizational skills.
