Job summary

Trinity Health MI in Brighton, MI, seeks an ED Case Manager to coordinate admission processes in our Level IV Trauma ED at Trinity Health Livingston Hospital.

You will review the clinical appropriateness of admission, communicate with the Patient Access team, and support discharge planning. A Michigan nursing license and case management experience are required; benefits accrue from day one.

Qualifications

• Current Michigan Nursing License.
• Associate degree in Nursing; BSN required within 5 years of employment in the role.
• Three (3) years recent experience in case management, utilization review or a related field.
• Case Management certification desirable: CMSA certification within three years or in progress.

Responsibilities

• Facilitates actions necessary for admitted or emergency center patients to receive the right care in the right place at the right time.
• Assess all patients that qualify as a readmission and determine reason for return; document readmission information/interventions in the Epic CM system.
• Identify barriers to discharging patients from the ED; develop strategies and coordinate with disciplines to address barriers to discharge.
• Consults with the ED physician to determine eligibility for admission and documents inpatient admission as required by hospital/insurance criteria.
• Consults to determine appropriate admission status (inpatient or observation) and level of care; document treatment plan and admission reasons.

• This position supports the 38-bed, Level IV Trauma Emergency Department at Trinity Health Livingston Hospital in Brighton, a state-of-the-art facility that opened in April 2026 and serves as Livingston County's only full-service hospital.

Miniumum Qualifications

• Current Michigan Nursing License
• Graduation from accredited nursing program: Minimum of Associates degree in Nursing, BSN required within 5 years from the date of employment in the role.
• Experience: Three (3) years recent experience in case management, utilization review or a related field.
• Certification: Case Management is desirable: Individuals serving as case managers for the health system shall obtain CMSA certification within three years from their date of employment in the role or be in the process of obtaining certification as a case manager.

What You Will Do

• Facilitates actions necessary for admitted or emergency center patients to receive the right care in the right place at the right time.
• Assess all patients that qualify as a readmission. Determine reason for return and implement alternative to admission plan when appropriate. Document readmission information/interventions in the Epic CM system.
• Identifies barriers to discharging patients from the E.D instead of admission as observation. Develops strategies and initiatives to address identified barriers. Work with other disciplines to assist in resolution of barriers to discharge. Report findings to Acute Care Flow Leadership team.
• Proactively consults with the EC physician to determine eligibility for admission. Facilitates appropriate documentation for inpatient admission as required by hospital and insurance criteria.
• Proactively consults with the EC physician to determine appropriate admission status (inpatient or observation) and level of care required. Facilitates appropriate documentation of reason for admission including the treatment plan as required by hospital and insurance criteria.
• Communicates medical necessity documentation requirements with attending physician as needed.
