Job summary

St. Croix Hospice in Manhattan, KS is seeking a Registered Nurse Case Manager to deliver holistic in-home hospice care.

You will develop personalized care plans, lead end-of-life conversations, and collaborate with a multidisciplinary team to manage symptoms and ensure comfort and dignity for patients and families. Responsibilities include conducting comprehensive assessments, coordinating medications and services, and rotating on-call duties.

Qualifications

• RN with an accredited nursing degree (ASN/BSN/MSN)
• 1+ years of recent acute care experience; hospice/home health experience preferred
• Conduct initial and ongoing comprehensive assessments to determine hospice needs
• Develop individualized care plans with patient and family, updating as needs evolve
• Rotate on-call duties and ensure seamless care transitions
• Must have reliable transportation, valid driver’s license, and proof of auto insurance

As a Registered Nurse Case Manager, you will serve as the cornerstone of patient care, delivering individualized, holistic support to patients in their homes.
