Kettering Home Care in Ohio is seeking a Registered Nurse Case Manager (RNCM) to lead one-on-one home-based care for patients, coordinating with physicians and an interdisciplinary team to ensure quality outcomes.

The RNCM will work at the top of their license, educate patients and families, manage caseloads, and ensure safety and compliance. Schedule: Monday–Friday, 8:30am–5:00pm with on-call rotation evenings/weekends/holidays.

Qualificações

• RN with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting; home care preferred.
• Valid driver's license and auto insurance in your name.
• Capable of all physical demands.

You will case manage and provide clinical care under the direction of the physician. You’ll educate patients and their family members and ensures the safety of the patient. You will work autonomously, so you need to be efficient in managing your time.
