Adaptive Home Health is seeking an RN Case Manager for field-based, full-time work in Huntsville, TX. You will own patient assessments, manage skilled visits including OASIS, and coordinate care across physicians, families, and the care team.

Qualifications

• Current Texas RN license.
• 3+ years of acute care nursing experience.
• Experience with wound care and infusions.
• CPR certification (BLS).
• Texas driver's license and field-based work readiness.
• Strong time management and organization in a high-autonomy environment.
• Clear, professional communication with patients, families, and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care, focusing on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians and care team.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination and timely follow-ups.
• Coordinate services to ensure the plan of care is executed.
• Administer medications and provide patient and caregiver teaching.
• Supervise LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality and safety.

• Role: Registered Nurse (RN) Case Manger – Home Health
• Compensation: $64.10 per point, plus mileage reimbursement
• Work Week: Full-Time
• Hours: Day shift-9:00 AM – 5:00 PM
• Schedule: Monday-Friday ( On Call rotation required)
• Location: Field-Based | Huntsville and surrounding area-Pod

What You Will Do

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care, with a focus on wound care and infusion therapy.
• Build and maintain a clear plan of care through consistent communication with physicians, care team members, patients, and families.
• Serve as the patients advocate and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination, timely follow-ups, and barrier resolution to keep patients progressing toward goals.
• Communicate changes in condition, obtain/track orders, and coordinate services (PT/OT/ST/MSW/HHA) to ensure the plan of care is executed.
• Administer medications and treatments as ordered and provide patient and caregiver teaching.
• Coordinate with internal clinical and operational support teams so that care is delivered reliably and safely.
• Provide supervision and guidance to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality, safety, and clinical outcomes.

Requirements

Must-haves

• Current Texas RN license
• 3+ years of acute care nursing experience
• Experienced with wound care and infusions
• CPR required (BLS)
• Current Texas drivers license and comfort with field-based work

Nice-to-haves

• Previous home health experience a plus
• Experience coordinating care for medically complex patients
