Procasemanagement is hiring Part-Time Registered Nurse – RN Case Managers in Colorado (Rifle and Grand Junction). You will conduct weekly in-home nursing visits, perform vitals, ensure home safety, and oversee caregivers.

The role emphasizes one-on-one patient care and improving quality of life for former Nuclear Weapons Workers with chronic or terminal illnesses. Candidates must be a graduate of a state-approved nursing program with an unrestricted RN license, BSN preferred, and at least two

Responsibilities

• Perform in-home nursing visits weekly (1–2 hours).

• Manage and coordinate care plans with caregivers and regional director.
• Ensure compliance with policies and nursing delegation guidelines.
• Participate in staff interviews and training as needed.
• Maintain accurate clinical documentation and chart completeness.

All our clients are former Nuclear Weapon's Workers with chronic/terminal illness due to exposure to toxic substances while working for the Government. The nursing visits are conducted weekly and typically last 1-2 hours per visit.
