Job summary

St. Croix Hospice in Grand Island, NE is seeking a Registered Nurse Case Manager to deliver 1:1 holistic home-based care. You will partner with an interdisciplinary team to develop tailored care plans, guide end-of-life discussions, and ensure comfort with professionalism and empathy.

The role requires RN licensure with an accredited nursing degree and at least 1 year of acute care experience, hospice or home health preferred. On-call duties and reliable transportation are part of the schedule.

As a Registered Nurse Case Manager, you will be the heart of our care model, providing 1:1 holistic support to patients and families in their homes. Qualifications

Registered Nurse Case Manager Requirements & Responsibilities:

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Complete initial and ongoing comprehensive assessments to determine hospice needs
• Develop individualized care plans with the patient and family, updating as needs evolve
• Administer medications and treatments as prescribed
• Educate patients and caregivers on care techniques, medications, diet, and symptom management
• Rotate on-call duties and ensure seamless care transitions
• Must have reliable transportation, valid driver’s license, and proof of auto insurance
