Job summary

Vibra Permanent Staffing is seeking an experienced Registered Nurse Case Manager for a direct hire, full-time role in a LTAC setting. You will coordinate individualized care plans with an interdisciplinary team and advocate for patients’ long-term recovery.

Ideal candidates hold an active RN license in the state or a compact state, have 1+ year CM experience, and are BLS certified with ACLS within 90 days of hire. A compassionate, team-focused mindset is essential.

Qualifications

• Active RN license in position state or compact state.
• 1+ year of Case Management experience preferred.
• BLS required; ACLS required within 90 days of hire.
• Strong clinical judgment and a compassionate, team-oriented mindset.

Responsibilities

• Provide direct care to critically ill or medically complex patients.
• Collaborate with physicians and interdisciplinary team to coordinate individualized care plans.
• Monitor patient progress, administer medications and treatments, and document clinical findings.
• Educate patients and families, and advocate for long-term health and recovery.
• Maintain compliance with policies, standards, and regulatory guidelines.
