Job summary

Lutheran Senior Services is seeking a Registered Nurse to provide and coordinate comprehensive resident care in a skilled nursing setting. The role requires current RN licensure in the practice state and CPR certification, with a preference for BSN and gerontological nursing experience.

You will work with an interdisciplinary team, document care plans, and monitor resident status while ensuring regulatory compliance and quality outcomes in a supportive environment.

Qualifications

  • RN licensure in state of practice.
  • Graduate of an accredited/approved nursing school.
  • BSN preferred.
  • CPR certification required.
  • Specialized training or experience in gerontological nursing preferred.

Responsibilities

  • Practice nursing within licensure and act within Nurse Practice Act.
  • Collaborate with interdisciplinary team for individualized plans of care.
  • Mix TPNs, hang first bag of blood, access/de-access ports, perform IV pushes.
  • Pass/oversee medication administration and IV therapy.
  • Maintain documentation, care plans, and MDS as applicable.
  • Utilize EMR for complete clinical documentation.
  • Observe resident status and coordinate with services for continuity of care.
  • Maintain safe, comfortable environment and assess hazards.
  • Oversee certified staff to meet EverTrue standards of care.
  • Complete rounds and document resident progress and changes.

Job Description:SummaryThe Registered Nurse (RN) performs nursing functions within the scope of practice and provides and/or coordinates care and services to meet the residents’ physical, psychosocial, and functional care needs.Job Responsibilities1. Practices nursing care within the scope of licensure and in accordance with the state Nurse Practice Act; Keeps a current active license; Follows state and federal licensure rules and regulations for the level of care in which they practice and in accordance with EverTrue clinical and administrative policies and procedures2. Works collaboratively with the interdisciplinary team in defining individualized plans of care for each resident; Works closely with nursing leadership to ensure regulatory compliance; Promotes individual and team learning and provides teaching and role modeling of professional nursing practices to staff for quality clinical care3. Mixes TPNs, hangs first bag of blood for infusions, accesses, and de-accesses ports, performs IV pushes, and carries out other duties consistent with the registered nurse licensure4. Passes medication and/or oversees the medication ordering and administration process; Administers medications and treatments to residents; monitors administration of I.V. fluids, supplemental nourishments, and medications when ordered; I.V. certified nurses may initiate I.V. therapy5. Maintains a working knowledge and understanding of the observation/assessment and documentation process for the level of licensure in which they work, including care and service plans and the MDS as applicable6. Utilizes the electronic medical record to capture full clinical documentation in accordance with policy and procedures7. Observes resident status as necessary to assure quality care; Coordinates with other resident services to assure continuity of care; Documents medical information and maintains detailed, timely, and accurate reports8. Maintains a safe, comfortable, and functional environment by assessing potential hazards; Implements practices to maintain optimum safety9. Oversees and directs the certified staff and others as assigned to ensure the EverTrue standards of care and services are provided10. Performs rounds according to established schedule; Documents resident progress, changes in condition, etc.11.
