Job summary

Prisma Health's Baptist Hospital Surgery Wound Care team is seeking an experienced Registered Nurse to provide specialized care for patients with chronic wounds. You will collaborate with physicians and the wound care team to deliver evidence-based treatments in a patient-centered environment.

The role requires an RN license and at least 2 years of nursing experience, with wound care and Wound Ostomy certification preferred.

Qualifications

• Associate degree in nursing required; BSN preferred.
• Minimum 2 years nursing experience with skin and wound care.
• RN license; Wound Ostomy certification preferred.

Responsibilities

• Assess and reassess the physical and educational needs of patients with chronic wounds.
• Coordinate care with wound care team to implement a cost-effective plan.
• Educate patients and caregivers on current advanced wound care modalities.
• Maintain EMR documentation of products and facility charges.
• Assist with hyperbaric therapy duties after required training.

Job Summary

Provides care for a specific patient population with various types of chronic wounds and their advanced wound care treatments. Serves as a resource for other health care professionals, patients, families and the community.

Essential Functions

• Under the guidance of a physician and/or a Certified Wound Care Clinician, is able to perform assessment/ reassessment of the physical and educational needs of patients with any chronic wound needing advanced wound care techniques. Communicates patient needs to appropriate members of the wound care team.Provides discharge teaching as required.Assists with referrals for studies, labs, procedures, and other physicians.
• Under the guidance of a physician and/or Certified Wound Care Clinician, formulates, implements and evaluates an individualized plan of care based on patient needs.Through consultation and collaboration with the members of the wound care team, coordinates care in an efficient, cost-effective manner using knowledge and understanding of current advanced wound care modalities and practices.
• Assists with education of patients and their caregivers on current evidence based advanced wound care modalities and practices.Assists with promoting knowledge of healthy practices in area of expertise throughout the community through education.
• Identifies actual/potential issues of resource management/patient care; works with wound care team to identify cost-effective alternatives; facilitates discharge.
• Keeps documentation via EMR current including accurate documentation of products and facility charges.
• Maintains safe, comfortable, clean and therapeutic environment for patients and families per established departmental polices.
• May perform hyperbaric oxygen therapy related duties, as needed, after completing the requiredhyperbaric training.
• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

• Education - Associate degree in Nursing. Bachelor’s degree in Nursing preferred.
• Experience - Two (2) years nursing experience. Experience in skin and wound care. Wound Vac experience preferred

• Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
• Certification in Wound and Ostomy preferred
