Princeton Transitional Care & Assisted Living, a joint venture involving Signature HealthCARE and Ballad Health, seeks an experienced RN to provide direct patient care and lead nursing staff within our new Princeton Transitional Care facility in the Tri-Cities region.

You will oversee ADLs, medication administration, and resident care plans while ensuring regulatory compliance and high-quality outcomes in a team-focused environment.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Proficiency with Microsoft Office applications.
• Familiarity with electronic health records (EHR) systems.

Responsabilités

• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

How you Will make a Difference:
