Job summary

Reliant Care Management, LLC in Ohio is seeking a Registered Nurse to provide patient care with empathy and patience. The role emphasizes care for residents, adherence to high standards, and collaboration with the healthcare team.

The RN will assess health needs, implement care plans, administer medications, monitor vitals, educate patients and families, document progress in EMR, and engage in ongoing professional development. A state RN license and 1 year of RN experience are preferred.

Qualifications

• Current unencumbered state license as a Registered Nurse.
• Minimum of 1 year of experience as a Registered Nurse preferred.

Responsibilities

• Assess patients’ health needs and develop and implement care plans.
• Administer medications and treatments as prescribed.
• Monitor patients’ vital signs and report changes to the appropriate staff member.
• Communicate with patients and families to provide education and support.
• Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment.
• Document patients’ progress and outcomes using electronic medical records.
• Stay up-to-date with current research and best practices in nursing.
• Participate in professional development opportunities to maintain and improve skills.
• Other duties as assigned.
