Stellar Health Group's Woburn Rehabilitation and Nursing Center is seeking a Registered Nurse (RN) to provide resident-centered care and collaborate with an interdisciplinary team. A $5,000 sign-on bonus is available for eligible applicants.

Qualifications

Job Summary

Woburn Rehabilitation and Nursing Center, a dynamic and innovative healthcare facility, is seeking a dedicated and compassionate Registered Nurse (RN) to join our team of caring professionals. As an RN, you will provide high-quality, resident-centered care while collaborating with an interdisciplinary team to develop individualized care plans and promote positive clinical outcomes. Responsibilities

• Provide direct nursing care to residents, including administering medications, obtaining vital signs, and monitoring residents' conditions.
• Develop and implement individualized resident care plans in collaboration with the interdisciplinary team.
• Observe residents and promptly identify and report changes in condition.
• Communicate effectively with residents, families, physicians, and other healthcare team members to ensure coordinated care.
• Accurately document resident care, clinical observations, and interventions.
• Participate in quality improvement initiatives and support the facility's commitment to excellence in resident care.
• Follow facility policies, procedures, and applicable state and federal regulations.
• Promote resident dignity, safety, independence, and overall well-being.
• Demonstrate a commitment to the facility's mission, values, and standards of excellence.

• Current, active, and unrestricted Massachusetts Registered Nurse (RN) license.
