Qualifications

• Must have a valid and active RN License in the state of practice.
• Ability to travel bringing care directly to patients' homes.
• Malpractice Insurance is required.

Responsibilities

• Conduct thorough assessments and ensure personalized, comprehensive care.
• Guide and educate patients and families on health maintenance.
• Provide assistance with Activities of Daily Living (ADLs) respectfully.
• Coordinate schedule directly with patients, ensuring comfort.
• Monitor and report any changes in patient symptoms or behavior.
• Administer medications and treatments as prescribed.
• Communicate with Case Managers and physicians for unified care.
• Maintain accurate patient logging in EMR system.
• Offer emotional support to patients and their families.
