Job summary

Trilogy in Ohio is seeking a Registered Nurse (RN) to provide direct, quality clinical care and serve as a Team Leader for patient care staff. The role emphasizes leading a team of direct care providers to ensure safe medication administration, care planning, and documentation in alignment with Health Campus policies.

You will use the nursing process to deliver care and ensure continuity from admission through discharge, visiting residents daily to observe status and communicate changes.

Qualifications

• Must hold a current, valid state RN license.
• Current CPR certification required.

Responsibilities

• Lead direct care team to ensure safe medication/treatment administration and proper documentation.
• Apply the nursing process to deliver ongoing patient care from admission to discharge.
• Perform daily resident visits to assess status and report changes to charge nurse/physician/Director of Nursing.
• Identify emergencies and initiate lifesaving actions in the absence of a physician.
• Complete required admin tasks including forms, reports, and charting.

WHAT WE'RE LOOKING FOR

The Registered Nurse (RN) is primarily responsible for providing direct, quality clinical care and serves as a Team Leader to patient care staff.

Key Responsibilities

• Lead a team of direct care providers to ensure appropriate execution of medications and treatments, documentation, family teaching, care planning and patient care conferencing in compliance with the Health Campus Policies and Procedures.
• Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.

Qualifications

• Must have and maintain a current, valid state RN license
• Current, valid CPR certification required
