TEEMA is seeking a dedicated Registered Nurse (CA) to join our care team in the San Jose area. The role focuses on delivering patient-centered nursing care, coordinating with physicians and the care team, and promoting health and wellness across diverse settings.

The ideal candidate has a CA RN license, a nursing degree, strong clinical skills, and the ability to travel to patient locations. Experience with home health or outpatient care is a plus, and compliance with HIPAA and infection control

Qualifications

  • Active RN license in CA.
  • Bachelor's or Associate's Degree in Nursing from an accredited program.
  • Strong clinical, assessment, and patient care skills.
  • Excellent communication, interpersonal, and patient education skills.
  • Ability and willingness to travel to patient locations, if required.
  • Current BLS and ACLS certifications, if applicable.
  • Valid driver's license.

Responsibilities

  • Provide patient-centered nursing care including assessment, planning, implementation, and evaluation.
  • Administer medications, treatments, and procedures according to care plans.
  • Monitor patients and document findings; report changes to the care team.
  • Educate patients and families on disease management and preventive care.
  • Collaborate with physicians and care team to coordinate patient care.
  • Develop and implement individualized care plans for complex patients.
  • Maintain accurate patient records complying with regulatory requirements.
  • Support both in-person and telehealth visits as applicable.
  • Adhere to infection control, safety, and HIPAA regulations.
  • Participate in continuing education and professional development.

Role Summary:

In this role, you will be a key member of the care team, delivering personalized nursing care, coordinating with healthcare professionals, and promoting overall patient health and wellness. Duties / Responsibilities:

Provide high-quality, patient-centered nursing care, including assessment, planning, implementation, and evaluation.

Administer medications, treatments, and procedures according to established protocols and patient care plans.

Monitor patients' conditions, document findings, and report changes to the care team promptly.

Educate patients and their families on disease management, health promotion, and preventive care.

Collaborate with the care team, physicians, and other healthcare professionals to coordinate and optimize patient care.

Assist in the development and implementation of individualized care plans for patients with complex needs.

Maintain accurate and timely patient records, ensuring compliance with all regulatory and organizational requirements.

Provide support for both in-person and telehealth visits, as applicable.

Ensure adherence to infection control, safety, and HIPAA regulations.

Participate in continuing education and professional development opportunities.

Perform other related duties as assigned.

Required Qualifications:

Active Registered Nurse (RN) license in CA.

Bachelor's or Associate's Degree in Nursing from an accredited program.

Strong clinical, assessment, and patient care skills.

Excellent communication, interpersonal, and patient education skills.

Ability and willingness to travel to patient locations, if required.

Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications, if applicable.

Valid driver's license.
