Job summary

Signature HealthCARE Rockwood Rehab & Wellness Center is seeking an experienced Registered Nurse to join our compassionate team in Rockwood, Tennessee. You will provide direct patient care, supervise nursing assistants, and collaborate with physicians to deliver high-quality outcomes.

We value teamwork, ongoing education, and patient-centered care. A current RN license and CPR/BLS are required; long-term care experience is preferred.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift.
• Monitor residents’ physical and emotional status; notify physician and responsible party of changes.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy and procedure.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.

• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
