Job summary

Trilogy is seeking aRegistered Nurse (RN) Team Leader to provide direct clinical care and mentor patient care staff on the Hudsonville, MI unit. You will lead a team to ensure accurate medication administration, documentation, and care planning throughout admission to discharge.

The ideal candidate holds an active RN license and CPR, embraces teamwork, and is committed to delivering compassionate, high-quality care in a supportive long-term care setting.

Qualifications

• Must hold and maintain a current, valid state RN license.
• Current CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure appropriate execution of medications and treatments, documentation, family teaching, care planning and patient care conferencing in compliance with policies.
• Utilize the nursing process to deliver patient care and ensure continuity from admission through discharge and return to family and community.
• Visit residents daily to observe and evaluate health status and notify the charge nurse/physician/Director of Nursing of changes.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc.

WHAT WE'RE LOOKING FOR

The Registered Nurse (RN) is primarily responsible for providing direct, quality clinical care and serves as a Team Leader to patient care staff.

Key Responsibilities

• Lead a team of direct care providers to ensure appropriate execution of medications and treatments, documentation, family teaching, care planning and patient care conferencing in compliance with the Health Campus Policies and Procedures.
• Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.
