Job summary

Signature HealthCARE in Muncie, IN seeks an experienced Registered Nurse to provide direct resident care, supervise nursing staff, and coordinate care plans within a supportive team. The role emphasizes patient-centered care, adherence to regulations, and quality documentation.

The position requires an active RN license in Indiana, at least 1 year of clinical nursing experience (long-term care preferred), and familiarity with EHR systems.

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Supervisory or charge nurse experience preferred.
• Strong communication and documentation skills.
• Familiarity with electronic health records (EHR).

Responsibilities

• Provide direct nursing care to residents, including medications, wound care, and skilled procedures.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides on shift per policies.
• Monitor residents, notify physician/party of any changes.
• Develop, implement, and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedure.
• Participate in QA activities and staff education.

How You Will Make a Difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What You Need To Make a Difference

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
