Job summary

Standing Stone Care & Rehabilitation Center is seeking an experienced RN to join our skilled nursing team. You will provide direct patient care, administer medications, and help residents with daily activities while maintaining dignity and safety.

Ideal candidates have an active RN license, 1+ year of clinical nursing experience (prefer long-term care) and familiarity with EHR systems and MS Office. This full-time role offers comprehensive benefits and a collaborative work environment.

Qualifications

• Active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems and MS Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with ADLs and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the shift per policy.
• Monitor residents' status and notify physician or family of changes.
• Develop, implement, and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing.

How you Will make a Difference:

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.

What you Need to make a Difference:

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
