Elite Care Management in Northwest Indiana seeks a part-time Home Health Visit Nurse to provide compassionate care in clients' homes. You will conduct assessments, administer medications, coordinate with care teams, and educate families on care plans.

Role Description

This is a part-time/PRN on-site role for a Visit Nurse at Elite Care Management. As a Home Health Visit Nurse, you will be responsible for providing compassionate and personalized care to patients in their homes. Your day-to-day tasks will include conducting assessments, administering medications, coordinating with other healthcare professionals, and educating patients and their families on care plans.
