Signature HealthCARE, LLC in Memphis, TN, is seeking an experienced Registered Nurse to join our on-site care team in a 140-bed facility. You will deliver direct patient care, oversee nursing assistants, and contribute to individual care plans with the interdisciplinary team.

The role emphasizes clinical judgment, documentation accuracy, and adherence to long-term care regulations. Strong teamwork and communication are essential as you support residents across shifts and participate in quality

Qualifications

• RN license in the state of practice is required.
• CPR/BLS certification preferred (AHA or Red Cross).
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role is a plus.
• Strong documentation and observation skills; knowledge of long-term care regulations.

Responsibilities

• Provide direct nursing care, medication administration, wound care, and procedures.
• Assist residents with ADLs, ensuring comfort, dignity and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor residents' status and notify physicians of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and staff education.
• Perform other duties as assigned by the Director of Nursing.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
