Job summary

Trinity Health is seeking an Ambulatory Triage Nurse for the Pulmonary Clinic on the Maywood campus. The role supports providers across the Maywood site and occasionally at Burr Ridge, Oakbrook, and Gottlieb sites.

The RN will assess health status, develop care plans, and coordinate care with the care team to meet patient goals. We value dedicated nurses with a BSN or ADN, Illinois RN licensure, and BLS certification, offering day shifts, full-time hours, and strong professional development

Qualifications

• Diploma from an accredited nursing program (BSN or ADN) required.
• BLS certification required prior to hire.
• Current RN license in the State of Illinois.
• 1–2 years of clinical experience.

Responsibilities

• Gathers pertinent data and information relative to patients' health status and needs and analyzes the assessment data to determine actual or potential diagnoses, problems or issues.
• Identifies expected outcomes or goals of care and collaborates with patients and families to plan care.
• Develops a plan that prescribes strategies to attain outcomes.
• Implements the individualized care plan and coordinates care with the healthcare team.
• Evaluates progress toward outcomes and adjusts the plan as needed.
• Applies evidenced-based approaches to care.

What you will do:

• Gathers pertinent data and information relative to patients' health status and needs and analyzes the assessment data to determine actual or potential diagnoses, problems or issues.
• Identifies expected outcomes or goals of care based on a plan to individualize care and in collaboration with patients, families, groups, communities or populations served.
• Develops a plan that prescribes strategies to attain expected outcomes or goals.
• Nurses systematically implement the individualized care plan, facilitates the coordination of care with inter and intra-professional colleagues and employs strategies for health teaching and promotion.
• Evaluates progress toward achieving defined outcomes/goals and change the plan if necessary.
• Implements evidenced-based approaches to care.

What you will need:

• A Diploma from an accredited school/college of nursing is required - BSN or ADN
• BLS provider (accredited by the American Heart Association) required prior to hire
• Current Registered Nurse License State of Illinois
• 1-2 years of previous experience in a clinical setting.

Compensation:

Salary Range: $31.62 - $50.79 per hour

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
