Job summary

Signature HealthCARE of East Louisville seeks an experienced RN to join our skilled nursing team in Louisville, KY. You will provide direct nursing care, supervise nursing aides, and collaborate with an interdisciplinary team.

The role requires a current RN license in the state, strong documentation skills, and familiarity with EHR systems. Prior LTC or supervisory experience is preferred, along with CPR/BLS certification.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
