Job summary

Signature HealthCARE LLC in Louisville, KY is seeking a qualified RN to join our Rockford Rehab & Wellness Center team. You will deliver direct nursing care, administer medications, and monitor residents while ensuring dignity, safety, and comfort.

You will supervise nursing assistants, document care, and collaborate with an interdisciplinary team to create individualized care plans. We value strong clinical judgment, experience in long-term care, and familiarity with EHR systems and Microsoft

Qualifications

  • Current, active RN license in the state of practice.
  • CPR/BLS certification preferred (American Heart Association or American Red Cross).
  • Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
  • Experience in a supervisory or charge nurse role, preferred.
  • Familiarity with electronic health records (EHR) systems.
  • Proficiency with Microsoft Office applications.

Responsibilities

  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
  • Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
  • Monitor and observe residents’ physical and emotional status; notify the attending physician of any change in condition.
  • Develop, implement, and review individualized resident care plans with the interdisciplinary team.
  • Complete accurate and timely clinical documentation per facility policy.
  • Coordinate resident admissions, transfers, and discharges.
  • Participate in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing.
