Job summary

Family First Healthcare Services in Florida is seeking compassionate caregivers for Home Health Care. You will conduct skilled nursing assessments and develop care plans, including medication administration and wound care, tailored to each patient.

This role emphasizes patient education and collaboration with the care team to promote independence and safety. Ideal candidates hold an active RN license, have 1+ years in home health, and are proficient with EMR systems such as ALORA Plus.

Qualifications

• Must be a graduate from an accredited School of Nursing.
• Must hold an active license in the state as a Registered Nurse.
• One or more years of experience in community/home health preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Working knowledge of home healthcare and required nursing documentation.
• Organization and time management skills; knowledge of management processes.
• Able to contribute to quality of care through communication with nursing managers and staff.
• Proficient with computers; EMR documentation with ALORA Plus preferred.
• Criminal background check required.
• Current CPR certification; online certification not accepted.
• Bilingual (English/Spanish) strongly preferred.

What You’ll Do:

• Provide skilled nursing and comprehensive assessments to establish individualized care plans. This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care
• Develop and maintain a therapeutic relationship by educating patients and caregivers on disease processes, medications, individualized treatment plans and plans of care, and promoting self-advocacy and independence.
• Utilize state-of-the-art technology to timely document, process and maintain all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
• Facilitate communication between the patient and the interdisciplinary team.

• 6. Should be skillful in organization, and in the principles of time management, and have knowledge of management processes.
• 7. Must be able to contribute to the quality of care being rendered, through constructive communication with nursing managers and staff.
• 8. Proficient with computers and EMR documentation experience with ALORA Plus preferred
• 9. Must have a criminal background check.
• 10. Must have a current CPR certification. Online certification is not accepted.
• 11. Bilingual (English/Spanish) strongly preferred

Environment:

Contact with clients under a wide variety of circumstances. Care is provided in client’s living environment with varying situations, including private homes, independent or assisted living or in an institution such as a hospital or long-term care facility. May be required to respond to emergency situations. Travel required. Position includes tasks that have the potential for exposure to blood/body fluids/tissues and other potentially infectious material such as body fluids/tissues.

Physical Requirements:

Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift up to 50 lbs. Corrected visual and hearing acuity required.
