Job summary

Trilogy Health Services, LLC in Lafayette, Indiana is seeking an experienced Registered Nurse (RN) to serve as a Team Leader and provide direct, quality clinical care to residents. The role emphasizes leadership of patient care staff and coordination of medications, treatments, and care planning.

The RN will use the nursing process to ensure continuity of care from admission through discharge, performing daily resident assessments and escalating changes to the appropriate team members.

Qualifications

• Must hold a current, valid state RN license.
• Current CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure proper medications, treatments, documentation, family teaching, care planning and patient care conferences in line with Policies and Procedures.
• Utilize the nursing process to deliver patient care and ensure continuity from admission to discharge and back to family/community.
• Visit residents daily to observe and assess physical and emotional status; notify charge nurse/physician/DON of changes in condition.
• Identify resident problems or emergencies and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing forms, reports, evaluations, studies, and charting as needed.

Position Overview

The Registered Nurse (RN) is primarily responsible for providing direct, quality clinical care and serves as a Team Leader to patient care staff.

Key Responsibilities

• Lead a team of direct care providers to ensure appropriate execution of medications and treatments, documentation, family teaching, care planning and patient care conferencing in compliance with the Health Campus Policies and Procedures.
• Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.

Qualifications
