Qualifications

Responsibilities

• Be a pillar of support: conduct thorough assessments for personalized, comprehensive care.
• Educate patients and families on health maintenance and disease prevention.
• Provide assistance with Activities of Daily Living as needed.
• Coordinate schedule with patients, align with care plans and preferences.
• Monitor changes in patient symptoms or behavior and report promptly.
• Administer medications and treatments as prescribed by the physician.
• Communicate with Case Managers and collaborating physicians to ensure a unified approach.
• Maintain accurate patient documentation in the EMR system.
• Offer emotional support to patients and family members.

Responsibilities

Qualifications

• A valid and active RN License in the state of practice.

Pay Range
