Signature HealthCARE of Elizabethtown is seeking a registered nurse for full-time, 12-hour night shifts (7p–7a) with weekend rotation. Benefits start within 30 days. You will deliver direct nursing care, supervise aides, and ensure safety and high-quality documentation in long-term care.

Required active RN license in the state of Kentucky, with 1+ year clinical nursing experience (long-term care preferred) and EHR proficiency. Competitive wages up to $45/hr, with comprehensive benefits.

Responsabilidades

• Provide direct nursing care to residents, including med administration, wound care, and skilled procedures.
• Assist residents with ADLs, ensuring comfort, dignity and safety.
• Supervise nursing assistants and medication aides during shift per policies.
• Monitor residents and notify physicians of changes in condition.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers and discharges.
• Participate in QA activities, staff education, regulatory surveys.

How you Will make a Difference:

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference:

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
