Job summary

Steel City Hospice is hiring an RN Case Manager to support Hermitage and surrounding counties. Join our growing team of compassionate clinicians dedicated to end-of-life care and family support. We offer competitive salaries and a generous benefits package.

Responsibilities include managing patient and family caseload, coordinating with the interdisciplinary team, and ensuring quality care from admission through discharge. Monday–Friday, 8a–5p, with on-call rotation as required.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission, planning, implementation and evaluation phases of the nursing process.
• Obtain data and assess factors affecting patient and family needs and initiate interventions by the interdisciplin ary team.
• Initiate communication with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and timely care plans.
• Supervise and document delivery of care by Home Health Aide per guidelines.

What You’ll Do

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
