Job summary

Trilogy Health Services is seeking an RN for Mill Pond Health Campus in Greencastle, IN to provide direct clinical care and serve as Team Leader for patient care staff. Responsibilities include leading care teams, documenting care, and coordinating with physicians to maintain continuity of care from admission to discharge.

The role requires active RN licensure and CPR certification, with a focus on leveraging the nursing process to evaluate and respond to residents' physical and emotional needs

Qualifications

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure appropriate execution of medications and treatments, documentation, family teaching, care planning and patient care conferencing in compliance with campus policies.
• Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
• Visit residents on assigned unit daily to observe and evaluate each resident's physical and emotional status and notify the charge nurse, physician, and/or Director of Nursing if a change occurs.
• Identify resident problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc.

POSITION OVERVIEW

The Registered Nurse (RN) is primarily responsible for providing direct, quality clinical care and serves as a Team Leader to patient care staff.

KEY RESPONSIBILITIES

• Lead a team of direct care providers to ensure appropriate execution of medications and treatments, documentation, family teaching, care planning and patient care conferencing in compliance with the Health Campus Policies and Procedures.
• Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
• Visit residents on assigned unit daily to observe and evaluate each resident's physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident's condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.

QUALIFICATIONS

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.
