Alternate Solutions Health Network seeks an experienced Registered Nurse to provide skilled nursing care in patients' homes, coordinating care with physicians and a multidisciplinary team. The role emphasizes autonomy, timely visits, and accurate documentation to support positive patient outcomes.
You will administer medications, educate families, and manage plans of care while adhering to state regulations and agency policies. A valid license and reliable transportation are required.
AGENCY: Southwest General Home Health
SCHEDULE: 3 working days per week including two weekdays and every other weekend. No on call requirements, holiday rotation including one working holiday and one on call holiday
As a Registered Nurse (RN) the work you do every day makes a difference in the lives of our patients. You’ll provide skilled nursing care in a patient's home as directed by the attending physician. You’ll work autonomously, so you need to be efficient in managing your time. You’ll be responsible for all practices and duties within the scope of practice outlined by the state.
